  • How do members earn Healthy Rewards credits?

    Healthy Reward credits are available to all members who receive a qualifying wellness service(e.g., flu shoot, mammogram, wellness visit, etc..).

  • Where are credits loaded?

    Credits are automatically loaded to their Access on Demand Mastercard wallet for the eligible wellness service within 60 days of the date of service. 

  • Where can members use their credits?

    • Online purchases through Nation Benifits online store. 
    • In-store purchases through participating retailers. 

  • Do credits expire?

    Unused Health Reward credits rollover each month and expire at the end of the calendar year (December 31*)

Healthy Rewards Timeline and Expectations

  1. Date of Service: Member completes activity
  2. Provider submits claims
  3. Reward automatically loads onto members card (30-60 days)
  4. Date Awarded: Reward sent to Nations(2 days)
  5. Vendor Active: Nations loads reward on Wallet (5-7 business days, loaded Tuesdays/Thursdays)

Inform member rewards take 2 weeks to load following outreach to our team to set expectations

Eligibility:

Rewards Members must:

  • Be an active Brand New Day Medicare Advantage plan member.


Earning Rewards:

All Rewards Members are eligible to earn the following rewards:

  1. Health Risk Assessment = earn $25
  2. Annual Wellness Visit + earn $50


Check with your doctor to see if you are due for any of the below screenings:

  • Diabetic retinal Exam = earn $10(completed with an Optometrist or Ophthalmologist)
  • Mammogram = earn $30
  • Colorectal cancer screening (one of the below)- earn $35
  • Colonoscopy
  • Sigmoidoscopy
  • Fecal Occult Blood Test
  • Flu shot = earn $10


Your eligibility for screening rewards will vary based on your doctor's recommendations. You can only earn rewards for the screenings you are due for:

  • Each of these rewards can only be earned once in 2023.
  • Rewards will not be fulfilled outside of your plan year; 2023 rewards will be issued until 12/31/2023.
  • No rewards exceptions will be issued after 1/31.2014.
  • Health Risk Assessment can be completed prior to your effective date; the reward will not be fulfilled until afer your effective date. All remaining rewards will not be fulfilled for activities completed prior to your plan effective date.
